When it comes to becoming a family child care daycare, you will need to make sure your that not only your home is prepared, but you have all your documents prepared as well. Keeping your documents organized and up to date is key! There is a lot of paperwork that goes into starting your family child care (in home daycare) and keeping it running. So, if you can keep yourself organized from the start you will be doing good. I like check lists. So, I have put together a checklist of the documents you will need to have for your pre-license visit.

Needed for children enrolled:

  • JFS 01234 Child Enrollment Form and Health Information for Child Care (this is updated annually)
  • JFS 01305 Child Medical Statement for Child Care (updated annually for children not in school)
  • Immunization records (updated annually with JFS 01305 form)
  • JFS 01225 Routine Trip Permission for Child Care (needed for every child; updated annually)
  • JFS 01236 Child Medical/Physical Care Plan for Child Care (needed for child with special requests for medical or needs; updated annually)
  • JFS 01217 Request for Administration of Medication for Child Care (Updated annually – filled out completely)
  • JFS 01218 Basic Infant Information for Child Care (updated every 3 months until child transitions out of infants)

Important forms needed for Family Child Care License

  • Disaster Plan
  • Evacuations (Fire Drill, Tornado Drill, Emergency Lockdown) (Completed monthly or quarterly)
  • Medical, Dental, and General Emergency Plan for Child Care (update when JFS updates the form) JFS 01242
  • Dental First Aid (update when JFS updates the form) JFS 01201 http://www.odjfs.state.oh.us/forms/num/JFS01201/pdf/
  • Plan of Operation for Child Care (updated when information changes) JFS 01250
  • Written Information / Policy & Procedure Handbook
  • Incident/Injury Report for Child Care (Keep these records on file for 1 year) JFS 01299
  • Request for Administration of Medication for Child Care (Updated annually – filled out completely) JFS 01217
  • Weekly Attendance for Child Care (Kept on file for 1 year or otherwise specified) JFS 01208
  • Liability Insurance Statement for Family Child Care Providers (need one for each family/child) JFS 01933

Other Documents….

Of course you being the owner of your family child care can come up with extra documents and forms that you may want to provide for your families. I do a daily sheet for my families that have kids that are younger so that way they are able to see what their child and/or children ate, when they went to the bathroom, sleep time, etc. Communication is key for parents and it helps them feel comfortable with dropping their kids off. I believe more is better than less!
